Overview of Position: Provides supportive services and assistance to the Director of Ryan White Services by facilitating peer audits, quarterly meetings, special projects, and other duties as assigned. Carries a case load of 90-95 clients and provides direct medical case management services to people living with HIV/AIDS (PLWHA). Case management duties include: initial assessments on new clients, annual assessments on existing clients as well as bi-annual mental health and substance abuse screenings, and quarterly treatment adherence counseling; assisting the provider with the monitoring of indicator measures to ensure they are met in the necessary time frame; implementing interventions; continuously communicating with clients to identify potential problems/needs, and working with the clients to meet those needs.

 

Qualifications:

                                                            Master's degree from an accredited college or university in a human services field related to Social Work, Public Health, Health Education, Counseling or a closely related field with at least one year of case management experience particularly in an outpatient setting working with PLWHA.

 

                                                            Bachelor's degree from an accredited college or university in a human services field related to Social Work, Public Health, Health Education, or Counseling with at least three years of experience in case management/direct service provision in an outpatient setting working with PLWHA.

 

                                                           

Essential Job Functions:                   Team Lead Duties

  • Provides assistance with peer audits to ensure programmatic requirements are met and documented accordingly
  • Assists with quarterly case management meetings
  • Supports goals and objectives of the program and promotes a supportive and productive environment
  • Completes special projects as requested
  • Performs other duties as assigned by Program Director

                                                            Case Management Duties

  • Responsible for the management of a client caseload of 90-95 HIV+ individuals.
  • Conducts a complete assessment of the client's health, financial and employment status, housing condition, treatment adherence, mental health/substance use, and health education/risk reduction.
  • Follows established case management guidelines in conducting annual, bi-annual, and quarterly assessments.
  • Helps clients obtain medications through the statewide ADAP and other drug assistance programs.
  • Makes referrals based on client's needs for mental health counseling, substance abuse counseling, housing or financial issues, etc.
  • Responsible for the maintenance and accuracy of the client's electronic health record.
  • Stays current on HIV/AIDS related studies and health related information.
  • Gives the client consistent and reliable HIV/AIDS information.
  • Utilizes resources within the community to ensure the program is payer of last resort.

                                                           

Knowledge, Skills & Abilities:

  • Working knowledge of Ryan White program requirements and HIV/AIDS Bureau indicators
  • Must be familiar with HIV/AIDS disease processes and should also possess skills in medical case management in a clinical outpatient setting serving PLWHA.
  • Knowledge of computer systems and software applications (CAREWare, SuccessEHS, Provide Enterprise, MS Word, Excel, etc.)
  • Strong attention to detail in addition to exemplifying the capacity to adhere to instructions and complete all assigned tasks in a timely manner.
  • Possession of strong written and oral communication and problem solving skills required.
  • High ethical standards and professionalism with a demonstrated ability to ensure all applicable policies and procedures pertaining to the usage of patient information is strictly adhered to and the utmost attention to confidentially is observed.
  • Ability to work in a high performance environment as well as take initiative and handle various tasks simultaneously while working efficiently, effectively, and independently under minimal supervision.
  • Ability to foster a team environment. Must be able to lead by example.
  • Ability to build and maintain a productive working relationship with medical case managers, clinic, and support service staff.
  • Valid driver's license and willingness to travel 10% of time.
  • Other duties as assigned by Program Director.
